Role of Epidermal Growth Factor Receptor to determine chemotherapy response of Cisplatin and 5FU in Head and Neck Squamous Cell Carcinoma

Background : Epidermal Growth Factor Receptor (EGFR) is the most important biomarker of the Head and Neck Squamous Cell Carcinoma (HNSCC) which associated with poor prognosis and survival. In advanced disease, Cisplatin and 5FU chemotherapy was expected to function as a part of multimodality therapy, but sometimes in many cases turns out to give poor response, especially for positive EGFR.

Objective : The purpose of this research was to determine relation between EGFR expression and chemotherapy response with Cisplatin and 5FU regimen on the Head and Neck Squamous Cell Carcinoma.

Methods : A total 30 patients of advanced HNSCC between May 2011 and December 2012 received neoadjuvant chemotherapy Cisplatin 100mg/m2 and 5FU 1000mg/m2 in 3 cycles, simultaneously with immunochemistry examination of EGFR . The interpretation was positive and negative for EGFR, and poor and good for response. Other data such as age, sex, stage, Squamous Cell Carcinoma subtype and grade were considered as on statistically as a confounding factor.

Results : EGFR expression was positive in 23 of 30 (76.7 %) patients and chemotherapy response was good in 5 of 30 (16.7 %) patients. There was no significant correlation between EGFR expression and response (p value = 0.068). The relative risk was 1.598 (CI 95% : 0.831- 3.072). There were no significant correlation between the confounding factor and response (all p value > 0.05).

Conclusion : There was no significant correlation between EGFR expression and chemotherapy response with Cisplatin and 5FU on advanced HNSCC. According to clinical importance, positive EGFR expression was associated with poorer response than negative EGFR.
